Revised Forms Containing MSRB’s New Address

As previously announced, the MSRB will be in a new location as of Monday, May 7, 2001. In connection with this new location, the following forms have been revised to include the new MSRB address: Forms G-36(OS), G-36(ARD), G-37/G-38, and G-37x. Please be mindful of the MSRB’s office location when submitting any of these forms. If you expect the forms to arrive at the MSRB’s office on May 7 or thereafter, use the revised forms with the new address.
